Evaluation Form Sample Pages

Students are given one of two surveys in class at the end of the term - Science/Engineering or HASS oriented. The front page of each survey allows the students to rate each of their instructors, as well as the subject itself, on a seven point scale with questions in a variety of categories. The back page of the surveys provides space for open-ended comments by the students.
