Locker Request Form

This form is to be used to request lockers for MIT student activities, student living groups, and team sports. To request an Activity Locker, your student activity must first be recognized by the Association of Student Activities (ASA), or by Sloan. Course lockers must be requested using the Course Locker Request Form.

	Your Username:		This must be your Athena (sometimes referred to as Kerberos or Eudora username.) You must have an Athena username to request an Athena locker.
