Organization Locker Request Form

Official MIT organizations are encouraged to request an Athena filesystem ("locker") for publishing information to the MIT community and the world. These lockers are paid for by the WCS group; there is no charge to you.

Use this form if you are an official MIT department and wish to use the locker to publish information on the World Wide Web.

It usually takes about 1-3 business days for these lockers to be created and set up. You will be notified via email when the locker is ready.

Your Information

Name:

E-mail:

Phone:

Name of MIT organization requesting this locker:

MIT affiliation of organization

administrative office
academic department
academic program
laboratory or research group
other (please specify):

If your organization does not appear in the MIT phone book (blue pages), please tell us how it fits into the MIT hierarchy, who runs the department, etc. (If we can not grant you a locker, consider requesting space from the organization which sponsors your project or group. We will be happy to set up a mailing list and subdirectory for you.)

Locker Information

Desired locker name (e.g., "admissions"):

  • Locker names may contain lower-case letters, numbers, and hyphens.
  • Locker names may not contain upper-case letters or "special" characters, such as spaces, quotes (", ', and `), underscores (_), ampersands (&), or slashes (\ and /).
  • Because of database limitations, "org" locker names must be shorter than 18 characters. Moreover, since your locker name will be a directory name and will be part of your URL, you probably want to choose a short name, so it's both easy to type and easy for people to remember. For example, "lcs" is both easier to type and easier to remember than "lab-4-comp-sci".

The locker owner is the person ultimately responsible for the locker. The locker owner does not necessarily have to be one of the people who is actually creating or editing the pages, but must have an Athena account. The locker owner should be a faculty or staff member who will be around MIT on a reasonably long-term basis.

Name of locker owner:

Athena username of locker owner:

@mit.edu

Phone of locker owner:

Locker owner's affiliation with MIT:

staff
faculty
other (please specify):

Athena usernames of everyone who will create/change files in this locker:

  • These must be Athena usernames.
  • Don't forget to include the locker owner if he/she should be on the list.
