MIT Campus Preview Weekend 2009

Registration

To register for CPW, please log in to your MyMIT account and click on the "Registration" link in the CPW portlet on the left.

Deadline to register: Thursday, April 2, 2009.

Parents: There is no separate registration form for parents/guardians. If your parent(s)/guardian(s) plan to attend, please complete the appropriate fields when you register.

Medical Release Form: All students attending CPW must submit a completed Medical Release Form (pdf) by April 2, 2009. If you will be under the age of 18 as of Thursday, April 16, 2009, the form must be completed and signed by your parent/guardian. You may mail the form to Campus Preview Weekend, MIT Office of Admissions, 77 Massachusetts Avenue, Room 3-108, Cambridge, MA 02139-4307, or fax it to 617-253-4076.

Athletics Waiver: If you wish to use MIT's athletic facilities, including our state-of-the-art Zeiger Athletic Center and olympic-size pool, you must complete an Athletics Waiver (pdf) and submit it via mail or fax with your Medical Release Form.

Change in plans: If your plans change after you've submitted the registration form, you can resubmit the form; we will only use the information from your latest submission.

Cancel registration: If you need to cancel your registration; return to the registration form and select "cancel registration" at the bottom of the form.
