MIT Sports Medicine Pre-Participation Forms
Required forms for Varsity Athletics participation:
1) MIT Student Medical Report
All new undergraduate, transfer, graduate, and readmitted MIT students must give MIT a medical history signed by a physician, have a complete physical exam, and document immunity against certain diseases.
2) Supplemental Medical History for Varsity Athletes
Students entering MIT who intend to participate in intercollegiate athletics
are required to have this form completed. Completion is recommended even if
students do not currently anticipate participation in intercollegiate athletics.
This form seeks additional information regarding your past medical history and
current health status. This form shall be presented to your health care provider
for his/her review and completion during your appointment for your physical
examination.

Additional forms required for varsity athlete participation by DAPER are currently undergoing software program reassignment and will be available in late summer.
