I have read, understand, agree to, and will abide by the Burton-Conner
Room Reservation Policy found at

I would like to request the following reservation:

Oct 21, 2000 (Saturday);  6:30pm - 2:00am;  Porter Room (Dining Hall).
Group: Caribbean Club (an MIT group)
There will be food at the event.
There will not be alcohol at the event.
Total number of people:  more than 150 (150 MIT, 100 others)
This event, a banquet, is the finale of Caribbean Weekend which includes a steel pan performance on Friday and a forum headed by a guest speaker discussing major issues that the Caribbean is currently facing such as the need for advanced technology. This banquet will feature different groups displaying cultural acts.  There will also be traditional Caribbean cuisine. This event is open to the wider MIT community.

I realize that in order to confirm a reservation, should my request 
be approved, I must submit any required fees to the House Manager,
and complete any other procedures specified by Burton-Conner Room 
Reservation Policy.  Unless I am told otherwise, I will assume the 
following fees must be paid as a minimum, dependent on the number of
people expected to attend the event:
     40 or fewer people - no charge
     41-100 people - $25.00
     101-150 people - $50.00
     more than 150 people - $75.00
