Col. Keesee gave a two part talk. He had a video showing numerous rockets exploding, and then he discussed the failure of the launch of Milstar 3. Here the Colonel shows one of the early rockets failing on the launch pad.
One of the Titan failures occurred because of high-temperature erosion of the wall of a booster rocket. The Milstar 3 failure was due to a single misplaced decimal point that did not get caught in the error-checking procedure.
