You must complete the CMSE Office Key Form. No other application is required. The MIT Key Request Form must be brought to E18-172 between 10AM to 2PM for a key to be issued.
All laboratory access (including access to the SEFs) is controlled by electronic locks activated by the user's MIT Card. Laboratory access cannot usually be granted to any individual who does not hold an MIT Card, except in some circumstances it may be possible to issue a "loaner" card valid for a limited time. For more details about this policy, please ask Ed Kruzel in room 13-2070 or call x3-6814.

Everyone who applies for SEF access must fill out an SEF Access Form. The supervisor of the specific facility that you will be using and your PI/Advisor both must sign the SEF Access Form. The list of supervisors appears below.

All keys transfers must be recorded with the CMSE office. If you have keys for a particular lab/office and you wish to transfer them to another person, you MUST fill out a CMSE Key Transfer Form and bring it to Ed Kruzel in room 13-2070.
You are responsible for the keys that you have been issued. All keys, upon end of use, must be returned to the key office in E18-172 unless the above key transfer will be done. Please notify Ed Kruzel of the key return (include your name, MIT ID#, and key #).
It is your responsibility to immediately report the loss or theft of all keys to the CMSE office in 13-2070 and to the MIT Key Office in E18-172.
