Procedures for Access to Building 13 Lab and Office Space

For an Office Key:

You must complete the CMSE Office Key Form. No other application is required. The MIT Key Request Form must be brought to E18-172 between 10AM to 2PM for a key to be issued.

All laboratory access (including access to the SEFs) is controlled by electronic locks activated by the user's MIT Card.   Laboratory access cannot usually be granted to any individual who does not hold an MIT Card, except in some circumstances it may be possible to issue a "loaner" card valid for a limited time.   For more details about this policy, please ask Ed Kruzel in room 13-2070 or call x3-6814.

For SEF Access:

Note: Professors can skip steps 1-6 and need only fill out the SEF Access Form.

  1. Pick up a copy and review the current Chemical Hygiene & Safety Plan, available in Room 13-2070.
  2. Complete both the Chemical Hygiene and Safety and Hazardous Waste Hypercourses located on the internet at: http://web.mit.edu/environment/training/
  3. Once you have taken the Chemical Hygiene and Safety and Managing Hazardous Waste hypercourses, print the Certificates of Completion.
  4. We ask that you watch the Chemical Hygiene & Safety Lecture that is now online at (pick the one that best matches your computer's download speed):

  5. Everyone who applies for SEF access must fill out an SEF Access Form. The supervisor of the specific facility that you will be using and your PI/Advisor both must sign the SEF Access Form. The list of supervisors appears below.

  6. SEF access will be granted in room 13-2070 when you present the completed SEF Access Form.

  5. Everyone applying for a Laboratory Key must fill out a Laboratory Key Form. The PI of the lab must sign the Laboratory Access Form.
  6. Laboratory access will be given in room 13-2070 once all internal CMSE forms are completed and presented in person.

Key Transfer Form:

All keys transfers must be recorded with the CMSE office. If you have keys for a particular lab/office and you wish to transfer them to another person, you MUST fill out a CMSE Key Transfer Form and bring it to Ed Kruzel in room 13-2070.

Returning Keys:

You are responsible for the keys that you have been issued. All keys, upon end of use, must be returned to the key office in E18-172 unless the above key transfer will be done. Please notify Ed Kruzel of the key return (include your name, MIT ID#, and key #).

Lost and Stolen Keys:

It is your responsibility to immediately report the loss or theft of all keys to the CMSE office in 13-2070 and to the MIT Key Office in E18-172.

Blank forms will also be available in paper holders on the wall outside of 13-2070 and will be color-coded:
