Here's what we learned about getting visa applications from the Burkina
Embassy in the USA: You must write a cover letter to them requesting visa
forms (which may be photocopied if you need more than one) and enclose a
SASE (self-addressed stamped envelope) for them to mail the visa forms back
to you in. They will not use their own postage, so you have to do the same
thing when you mail your passports in for visa stamps. You have to send
them a SASE or FEDEX label for them to ship back to you on your account.

In the USA, send requests to:

Attn: Visa Dept
Embassy of Burkina Faso
2340 Massachusetts Ave NW
Washington, DC 20008
