All new investigators and their study staff must complete the MIT appointment process prior to the start of their research at the Institute. The appointment process will require meeting with CRC administration.
- Call or emailthe CRC Administrative Office (617)253-3091 to schedule an appointment:
- Bring the following items with you to the appointment:
- Passport or two other forms of ID(driver's license and either SS Card or original birth certificate)
- Immunization Record to confirm PPD testing within the last year
- Confirmation of IRB Human Subject Training and HIPPA training
- IRB CITI Training Certificate (within the last three years) and HIPAA Training Certificate
- or Certificate from MIT COUHES Human Subject and HIPPA Training web course
- NIH Bio sketch, CV or Resume
- The following forms will be completed at the appointment:
- I-9 Form - requires forms of ID specified above
- Staff Appointment Form - address, phone, etc.
- Inventions and Proprietary Information Agreement - requires CRC witness
- Confidentiality of Subject Information Form - signature
- Safety Training Form - signature
- MIT Training scheduled(if applicable) - MUST be completed before working at CRC
- MIT Blood Borne Pathogen Training
It is noteworthy to mention that this process usually requires two visits to the MIT CRC.
- In addition to completing the appointment forms, it is usually advised that a protocol amendment be submitted to include this individual (s) on the protocol.
- Once all appointment forms have been processed and approved, the appropriate party will be notified via email.
- Remember that an appointment request is not cleared to work on a protocol at the MIT CRC until the entire appointment process is completed and the amendment to include the individual(s) on the protocol has been approved (in most cases).
- The PPD test can be administered by the CRC Nursing Staff if need on your appointment date.
