In addition to the appointment process, all new licensed researchers: PhD's, physicians, physician assistants, nurse practitioners and nurses must be credentialed by the MIT Medical Department.
- Credentialing performed by centralized verification organization, Med Advantage. .
- Credentialing Application Packet sent directly to the appointee's mailing address
- Application must be completed by Appointee
- Documents personal,educational, and professional information
- some parts of the application may not pertain to each appointee and it is permissible to leave an area blank or explain why it is blank if an explanation is needed.
- The appointee will send the completed packet directly to Med Advantage.
- Process typically takes about eight weeks
- The appointee will send a copy of the completed packet to CRC Administration Office before mailing
- The appointee will be notified by the CRC when your application has been accepted

- CRC Delineation of Privileges Form
- Form must be completed and signed by Appointee
- The appointee will return the signed form to MIT Medical
- Upon Verification from Med Advantage, Physicians will be presented to the MIT Medical Credentials Committee to grant MIT CRC Privileges
- Committee meets monthly.
- Nurse practitioners, physician assistants and nurses may start work at the CRC when MIT appointment is finalized but must complete the credentialing packet within 30 days.
- Reappointment applications sent by Med Advantage every other year.
Please note that the CRC Administrative Assistant and the CRC Administrative Officer are available to help with any questions/problems that may arise during either the appointment or credentialing process. The status of each appointment in process is monitored daily.
