2008 MIT SUMMER DAY CAMP APPLICATION

When you are ready to apply for a space in the 2008 MIT Summer Day Camp, download and complete the following forms.

All forms must be complete in order to process your application. Incomplete application packages will be returned:

• Camper Application Form (Word, PDF)
• Health Form (Word, PDF)*
• Medication/Allergy Form, if necessary (Word, PDF)*
• CIT Application Form (Word, PDF)* (For campers 14 to 16 years of age)

*Health Form and Medication/Allergy Form must be submitted with registration.

Your camper will be enrolled in the order in which the application is received, space permitting. Confirmation of your enrollment status will be sent to you by mail within two weeks of when it is received. Your registration fee is applied to total tuition costs.

At the present time, we are unable to accept forms emailed to Day Camp.
