Guidelines
for Pre-ER Decontamination after Laboratory Exposures

Filing Process for Incident Forms:
Just 1 folder
for the entire year, no more filing by month just put the incident
form in the appropriate year. This requires we all follow the same
file naming format:
Mandatory
file naming format: Yearmonthday_location_description
(i.e. 20080530_56-314_HF spill; 20080530_bldg56-3rd fl_odor day
call)
Process
Reminders:
- Must fill
out an Incident/Day Call form after responding to any incident
or day call.
- Be sure that
you are using the most recent version of the form; location: M:
EHS Response/Current Incident Report Form.
- Fill out
both sides of the form and if you know the dept
please note it.
- Save the
form with the above file naming format in the “To Be Entered
in SAP Incident Reports” folder (on the M drive in EHS Response
folder).
- You can use
paper copies of the form (the front desk as some), but we recommend
that you scan and pdf (by using the front desk copier) the form
and save it with the naming format in the year folder. The reason
for this is the paper forms will only be entered directly into
SAP (the system of record for all incidents and day calls) and
if you need to reference it in the future you will have to run
a report using brio. And though we hope to correct this, the Brio
reports currently do not include all the details from the form.
