Injury Reporting Procedures on 9/18/2008 - 9/19/2008 during SAP System Outage:

If an injury occurs between 6:00 p.m. on Thursday, September 18, 2008 and 9:00 a.m. on Friday, September 19, 2008, please fill out a Supervisor's Injury Report and e-mail a copy to the OSHA record-keeper:  sma@mit.edu 

• For a more serious accident involving injury to 3 or more people, or one which results in death, please contact the MIT Operations center by calling (617) 253-1500, or (3-1500) and they will page the EHS Office. You will then need to submit your supervisor's report of injury within 8 hours of the accident. If you fill out the report electronically, please email it to the OSHA record-keeper:  sma@mit.edu.  If you print out the report and fill it out manually, please fax it to the EHS Office at 617-258-6831 (MIT operations must still be contacted (617) 253-1500 (3-1500))  

• For accidents that do not meet the above mentioned criteria, please submit your supervisor's injury report with 24 hours of the accident. To do so, you can either wait to access the SAP Supervisor's Injury Report system after 9:00 a.m. on 9/19, or you can fill out the an electronic version of the form and e-mail a copy to the OSHA record-keeper:  sma@mit.edu or print it out and fax it to the EHS Office at 617-258-6831.
