reporting an Occupational Injury or Illness

The MIT Safety Program encourages everyone to report safety hazards. Depending on the nature and location of the hazard, this may be reported to your supervisor, Facilities or another responsible MIT department. If an accident occurs because of a unsafe condition, please report this so that it can be repaired before anyone else is injured.

Safety hazards inside a building:

Contact the Department of Facilities to repair a safety hazard using their web form.

They will process these requests faster than if we email or call the zone about a safety hazard.

To report an imminently hazardous situation that Facilities can help with, call FIXIT ext 3-4948 and select the emergency option on the voice mail menu.

If the floor is wet or slippery in the corridor, stairway or other public area inside a building, contact the Building Services Dept cust-coach@mit.edu

If the hazard is in a Classroom or Lecture Hall, contact 253-4788 or schedules@mit.edu

If the hazard is in an Athletics facility or field, contact the Athletic Dept athletics@mit.edu

Safety Hazards outside buildings:

To report hazards for pedestrians on steps, sidewalks and lawns inside the main campus, contact the Facilities Grounds Department Plant-grounds-coaches@mit.edu

Safety Hazards in Parking lots and garages, contact lrbrutti@mit.edu

Safety Hazards related to Construction or Renovation, use the contact us with the subject Safety and describe the hazard.

Safety Hazards related to traffic signals, cross-walks, streets and vehicular traffic

If the safety hazard is about Mass Ave, Vassar, Albany or other public street contact:

Cambridge Department of Traffic, Parking, and Transportation at 617-349-4700

If the safety concern is about a crosswalk or traffic signal on Memorial Drive, contact the MDC:

If there is a safety hazard with a crosswalk or fire lane within campus, on MIT owned and leased streets (Amherst Alley, Carleton St, etc), contact:

Kelley Brown, Senior Project Manager, Facilities Department at kbrown@mit.edu

If vehicles are creating a hazard, contact:

MIT Police Dept (x3-1212)

If the safety hazard doesn't fit into any of the above categories, use the contact us with the subject Safety and describe the hazard.
