EHS-MS Awareness Questionnaire

To better manage environment, health and safety (EHS) issues at the Institute, the EHS Office is gathering information to measure general awareness about MIT's Environment Health, and Safety Management System (EHS-MS). The questions are designed to gauge how successful we've been at increasing your awareness of the EHS-MS.

This questionnaire is MIT certificate protected but the data gathered will be anonymous.  By participating your name will be entered into a drawing for a gift certificate to thank you for your input but your name will not be tied in any way to your answers to ensure anonymity of your responses.

We appreciate you taking the time to respond and your answers will help guide us as we continue to integrate the EHS-MS into departments, labs, and centers.
