Invitation Letter Request Form

Below is the format of the invitation letter. Please complete the spaces below and click on the submit button. Invitation letters can be picked up in the International Students Office (room 5-133).

If you will be financially responsible for your visiting relative during his/her stay in the US, then please mail or drop off documentation of support (such as a bank statement) to the International Students Office. Your letter will not be processed without the documentation.
