Template 05: Simple Form
The text in this template is used as an example. This form is laid out using an HTML table. Be sure the text in your tables is enclosed in opening and closing paragraph <p></p> tags. Use "nowrap" tags to keep text in cells from wrapping. Note that the "links" in the left nav are optional and should either be deleted or customized to meet your needs.
