Is this jack activated?

   Yes   No

 

Since you already have a jack, we need to know if it has been activated. The jack is likely to be activated if it has ever had a computer or printer plugged in there before, and if you see a jack number on the jack housing.

If you do see a number on the jack housing, please write it down. If the number is unclear, or seems to have been changed, or is nonexistent, we will ask you later to send us a floor plan indicating where the person performing the installation can expect to find the jack.
