Hotel Reservations

A limited number of rooms have been blocked at the Westin Maui on Kaanapali Beach, Lahaina, Maui -- was placed atop the America's Best Beaches List in 2003. 

Reservations must be made before November 25, 2005 to ensure a space in this block. When making your reservations by telephone, please identify yourself as being with the 8th US-Japan Symposium on Drug Delivery Systems/MIT, to receive the preferred group rates. Reservations can be made at the special conference rates of $170.00 and $210.00 per night. This special rate is offered at a first come, first served basis.  Reservations received after November 25, 2005, or after all rooms in this group block are sold, are subject to Hotel space availability at the Group rate.  Please make your reservations early to be eligible for the limited group block and special conference rate.

Please use the reservation form provided here to fax your hotel reservation request. Reservations are made through the Westin Maui - Reservations Center Honolulu Office (contact information is listed at the bottom of the hotel reservation request form), but you will be staying at the Westin Maui at Kaanapali Beach, Lahaina, on the island of Maui (not Honolulu).

We have provided a PDF reservation form to ensure that your fax request is clear and readable upon fax. The form requires Adobe Acrobat Reader version 4 or higher to fill out on screen. You are invited to fill it out on screen, or you may print the file and fill it out by hand.

To fill out the form on screen using Adobe Acrobat Reader follow these simple steps:
