Our Medical Records Service keeps patient records for all
MIT Medical services, except Dental and Mental Health. For
access to those records, please contact the services directly.
Federal regulations require that you submit a written request
for access, to or copies of, your MIT Medical records and
X-rays.
To review your medical record:

To obtain a copy of your medical record:
Obtaining a copy of your medical record requires the completion
of an Authorization for Use and
Disclosure of Protected Health Information (pdf). Download and complete the form, and
then fax (617-258-0884) or mail it to the Medical Records
Service (Bldg. E23-023, 77 Massachusetts Ave. Cambridge,
MA 02139). Please
be advised that there may be a fee for the release of the
medical record.
To have your medical record transferred from another facility
to MIT Medical:
Download and complete the form Request
for Protected Health Information to MIT Medical (pdf) and
forward it to the facility you wish to obtain your medical
record from.
To have your Mental Health medical record transferred
from another facility to MIT Medical:
Download and complete the form Request
for Protected Health Information to MIT Mental Health Service (pdf) and
forward it to the facility you wish to obtain your medical
record from.
To request an amendment to your records:
Complete the form Request
for Amendment of Protected Health Information, specifying
which information you want amended, sign it, and mail or
fax it to the Medical Records Service. We'll notify you
when a decision has been made.
