MIT Medical's Pediatrics Service is committed to understanding
your concerns and treating your children with careful attention
to their individual needs and medical histories.
We encourage you to choose a personal
clinician for your children and set up an introductory
appointment. Please complete a history
form (pdf) and bring
it with you to this first appointment. The pediatric history
form is also available in Chinese, Japanese,
and Korean. Having
this information will help our clinicians provide your
child with the best possible care. Our clinicians, nurse
practitioners, and other pediatric staff stress prevention
and education, while providing a full range of services
for children and adolescents, including:
- Treatment of sickness or injuries
- Vaccines and immunizations
- Routine physicals and check-ups
- Lactation consulting for nursing mothers
- 24-hour on-call response
All children who will attend school in Massachusetts need
to submit a healthcare
provider's exam form and a certificate
of immunization to the school before he or she may attend
classes. If you are coming from outside the state or the
country, your local clinician can complete and sign these
forms before you arrive.
The Centers for Disease Control and Prevention produce vaccine
information statements (VIS)
that explain the benefits and risks for each vaccination
your child receives. MIT Medical clinicians will give you
the appropriate VIS when your child receives a vaccination,
but they are also available online in languages other than
English through the Immunization
Action Coalition.
