• Bruce Eckel has written a number of great books, some of which are online for free.
• refcards.com has a few handy refcards.

C/C++

• Steve Summit -- He maintains the comp.lang.c FAQ and has good notes on C. Required reading for a C programmer.
• Standard C documentation by P.J. Plauger. Plauger's company Dinkumware used to have a similarly great C++ reference online but sadly it's been taken down.
• Lysator C Archive
• cplusplus.com includes a library reference.
• Bjarne Stroustrup -- the inventor of C++
• The C++ FAQ is essential reading.
• I've mirrored Bruce Eckel's excellent Thinking in C++ here: (volume one, volume two).
• C++ Annotations -- I haven't looked at this one for a while.
• Incompatibilities between ISO C and ISO C++ -- by David Tribble
