Interphase 2009 Participant Forms


Please read the following forms:

You will need to complete and return forms 1, 2, and 4 by Friday, June 5th. You can fax, email (gonzalsx@mit.edu), or mail the forms above to our office. If any documents are illegible, we will notify you to send a new copy.

In addition to the forms above, all Interphase participants MUST submit the following items (due by Friday, June 5th):

  1. Two (2) black &white or color passport-size photographs (focus on full face, head to chin). The photos are specifically for the Interphase program, and are in addition to any other photos that MIT has requested. You can send these in pdf format, but please DO NOT send faxed photos.  

  2. A short, typed introduction about yourself (125 words or less).  This is your self-introduction to your fellow Interphasers.  Be creative or factual, funny or serious.  Your introductory paragraphs should be e-mailed to the administrative assistant, Sandy Gonzalez, at gonzalsx@mit.edu.

We will compile the photos and self-introductions into a booklet that will be shared with all Interphasers and the Interphase staff.

PE Requirements Summary


Summer 2009 Physical Education Interphase Options: 
Registration: Monday, May 18-May 25. First come, First serve.

Earn points this summer through swimming, weight training, or sailing! Register at http://mitpe.com

All Interphase students must sign up for swimming unless you are certain you can test out and swim 100 yards. If you are sure you can complete the swim test, you may elect to sign up for sailing or weight training instead. The Interphase program will cover the cost of one course – your student account will not be charged. Your student account will be charged the course fee if you elect to take additional courses. Students must complete 8 hours of instruction to earn two points toward eight point General Institute Requirement.
