Does the project include the use of human subjects, whether biological, psychological or sociological?
Is
NIH human subject training required?
Is an approved protocol required?
Does the project include use of animals?
An appropriate
protocol for the use of laboratory animals must be submitted to and approved
by the Committee on Animal Care.
The protocol number and most recent date of approval should be noted on the Proposal Summary Form.
Does the project involve recombinant DNA?
All projects which
involve recombinant DNA technology must be registered with and approved by the
MIT
Biosafety Program.
The date of approval must be noted on the Proposal Summary Form.
Does the project involve radioactive materials?
All project which
involve radioactive materials must be approved by Radiation
Protection Program.
The date of approval must be noted on the Proposal Summary Form.
Does the project international contractual commitments or significant activity in a foreign country?
Projects which will involve international contractual commitments or significant activity in a foreign country must be reviewed and approved by the Vice President for Research and Associate Provost, who will refer the proposal to the Committee on International Institutional Commitments as necessary.
Does the project include rental of space or construction or renovation of MIT property?
Requests for new space, space assignments, or requests for major alterations and renovations of existing spaces must be approved by the Committee for Review of Space Planning.
Is the proposal being submitted to a private foundation?
Proposals to be
submitted to private foundations must be cleared by the Office of Foundation
Relations.
The Proposal Summary Form should indicate who provided clearance and when clearance was received.
