Reviewing proofs

Selection of source and independent contractor forms
You'll need to complete a selection of source form when purchasing goods over $5,000. If you're working with a vendor who is new to MIT and who does not have a federal ID number, the completion of an independent contractor form is required to enter the vendor into SAP. An MIT certificate is required to access these forms.
