REENTERING STUDENTS (AFTER AN ABSENCE) PRE-REGISTRATION Undergraduates must be readmitted through Counseling and Support Services, Room 5-106. Graduate Students must be readmitted through their department and the MIT Admissions Office, Room 3-103. 1. Once readmitted, go immediately to the Registrar's Office, Room E19-335, and complete pre-registration; $40 late fee after January 20 if no extenuating circumstances. International Students: All International students must obtain clearance from the International Students Office before picking up Registration Material. Go to the International Students Office, Room 5-106, or call 253-3795 to make an appointment with one of the International Student Advisors. Bring all your visa documents -- passport, I-20, and I-94 forms -- to the session because the International Students Office is required to have updated information about your status. After the session you will be cleared for registration. Updated Medical Report After a year or more of not being registered at MIT, you must file a complete and up-to-date Medical Report, including a physical exam and documentation of all immunizations, with the Medical Department. A late fee of $80 is charged if the Medical Report was not received by January 23, 1998. For further information, contact the Health Screening Unit, E23-177, x3-1777. MIT ID Card You may have a picture taken for the MIT ID Card at one of two locations: the SSC Card Office, 11-120, open Monday through Thursday, 9:00 a.m. to 5:30 p.m. and Friday, 10:00 a.m. to 5:30 p.m.; OR the MIT Card Office, E32-121, open Monday through Friday, 9:00 a.m. to 5:30 p.m. You must present a valid picture ID, preferably a passport or current driver's license. REGISTRATION DAY - MONDAY, FEBRUARY 2 Meet With Your Faculty Advisor or Registration Officer 1. Meet with your advisor on Registration Day, February 2, between 9:00 a.m. and 4:00 p.m. to discuss your Spring Term registration plans. NOTE: Undergraduate students who entered MIT prior to September 1988 and are returning following an absence must meet the current General Institute degree requirements rather than those that applied when they entered. If you and your advisor wish to make additions or deletions to your program, these changes may be made directly on the three-part Registration Form. 2. Make any changes in your address/phone information and sign the Registration Form. 3. Your advisor will sign the Registration Form; make sure he or she gives you both the white and yellow copies, unseparated. Once your advisor has signed the Reg Form, all subsequent changes to your program require Add/Drop forms approved by your Advisor/ Registration Officer. 4. Take the unseparated white and yellow copies to du Pont Gym. Turn In Your Registration Form and Receive the Validated Yellow Copy 1. Go to du Pont Gym between 9:00 a.m. and 4:00 p.m. 2. Follow signs to the appropriate table and turn in the unseparated white and yellow copies of the signed Registration Form; receive the validated yellow copy. Undergraduates only: You may request a new Class Schedule at du Pont. Make schedule requests directly on the unseparated white and yellow copies of your Registration Form and turn the form in at the "schedule request" table in du Pont. Your new schedule will be available for pick-up the next morning (Feb. 3) at the Student Services Center, 11-120. Address Information Students have responsibility for providing accurate address information and updating it immediately on WebSIS if there are any changes during the term. This information appears in the on-line directories, including the World Wide Web, and is used for all official Institute mailings. See page 13 to request that address and other information not be made publicly available. 7
