
Use these forms to request an H-1B for a scholar who will be new to MIT, or who is currently at MIT in a different visa status. If the scholar is already in H-1B status at MIT, you must complete the Forms H for Extension of H-1B Status.
Please note! There are FOUR forms listed below. Forms 1, 2, and 3 should be completed by the Department, Laboratory, or Center. Form 4 should be completed by the prospective H-1B scholar.

Please type the required information on the fillable forms, then print them and provide original signatures where indicated. If you cannot complete the fillable forms on-line, we request that you print the blank forms and type the requested information.
Do not submit any forms to the ISchO via the Internet or as attachments to e-mail. The department, laboratory, or center must submit all the required H forms and any requested supporting documentation directly to the ISchO in hard copy.
All requested items must be completed.
The "information about the prospective scholar" must not be sent directly to the ISchO by the scholar as it must be reviewed at the departmental level and attached to other application materials provided by the department.
Any requested supporting documentation must be attached.
Due to changes in U.S. government practices, it is imperative that the names of scholars and family members be written on visa application documents exactly as they appear in their passports. Birth dates and name spellings are not always clear when handwritten. We request that all forms be typed/printed when possible. Please verify all information before submitting the application packet to the ISchO.
