Forms H - For Extension of H-1B Status

Use these forms to request an extension of status for scholars who are already at MIT and already holding H-1B status. If the scholar will be new to MIT, you must complete the Forms H for Prospective H-1B Scholars.

Please type the required information on the fillable forms listed below, then print them and provide original signatures where indicated. If you cannot complete the fillable forms on-line, please print the blank forms and type the requested information.

Do not submit any forms to the ISchO via the Internet or as e-mail attachments. The department, laboratory, or center must submit all the required H forms and any requested supporting documentation directly to the ISchO in hard copy. An MIT certificate is needed to access the forms. Please complete all three forms below.

Please Remember the Following:

• All requested items must be completed. Please be sure to provide the terms of appointment (title, salary, etc.) as of the start date of the extension. This is particularly important if there will be any changes to the appointment.
• Any requested supporting documentation must be attached.
• Due to changes in U.S. government practices, it is imperative that the names of scholars and family members be written on visa applications exactly as they appear in their passports. Birth dates and name spellings are not always clear when handwritten. We request that all forms be typed/printed when possible. Please verify all information before submitting the application packet to the ISchO.
