Biographic Record


Legal Name

The full Legal Name (including suffix, if any) appears on your MIT transcript, enrollment certifications, diplomas, and other official records. It often includes full middle name rather than just middle initial but the choice is yours.

To change your Legal Name you must complete a Change of Legal Name Form (available in the Student Services Center or Registrar's Office) and provide evidence such as a certified copy of a court order, a marriage certificate, or a dissolution decree reflecting the new name in full. Minor changes in names (e.g., spelling corrections) require documentation such as a current driver's license with photograph or a passport.

Administrative Name

The Administrative Name displays on MIT internal records such as class lists; on your status of registration report, grade report, loan note, and bursar's bill; and in the printed and electronic directories. To change the first and/or middle name of your Administrative Name, type the change in the text box provided. To change your family name you must follow the procedure above for a change of Legal Name.

Birthdate

Your birthdate displays on your transcript. To change your birthdate you must bring evidence such as a birth certificate or driver's license to the Student Services Center or Registrar's Office.

U.S. Social Security Number

Your social security number displays on your enrollment certifications and financial documents. You must change your social security number in person at the Student Services Center or Registrar's Office and provide evidence reflecting your social security number such as a current driver's license with photograph.

Citizenship/Immigration Status

If you are changing from an International Student to a U.S. Permanent Resident or a Citizen you need to go to the International Students Office, 5-106, and provide proof of your permanent residency or citizenship.

To change from Permanent Resident to Citizen you should go to the Student Services Center or Registrar's Office and provide evidence of your citizenship such as a passport or naturalization papers.

ILG Affiliation

If you are, or have been, affiliated with a Fraternity, Sorority, or Independent Living Group, regardless of your current address, you may use this field to update your affiliation.

Next Regular Term

Fall and Spring Terms are considered regular terms. Summer and IAP are not.

Admission Basis - Graduate Students and Undergraduate Transfers

The Previous College/University Attended means the most recent college you attended as a matriculating student (whether or not you received a degree). This attendance should be the most recent prior to your first semester of enrollment at MIT as a regular MIT student (not as a special or cross-registered student).

Please enter the complete English version of the name - do not abbreviate. For graduate students who entered MIT as freshmen, this would be blank and you should make sure that the High School you attended is correct.

Admission Basis - Entering Freshmen and Re-admitted Undergrads

High School Attended means the high school attended prior to initial enrollment at MIT.
