Dear NAME,

Recently, you received a letter from Dean Mahoney noting that one of
the rites of passage into college is the annual, national First-Year
Student Survey, sponsored by the Cooperative Institutional Research
Program (CIRP) at UCLA.  The letter briefly described the survey, uses
of the data that it collects, and noted that it would be administered
online, in August, at which time we would send you the link to the
survey.

It is now time for the survey.  Your connection to the survey is made
simply by clicking on the link below:

This link will take you directly to the survey.  If you experience any
problems with it, please contact me by replying to this message, or by
calling (413) 585-3021.  Students who respond to the survey will be
entered into three weekly drawings for three prizes each of $25
OneCard credits.  The earlier you respond, the more chances you have
to win.

All of your responses are secure, will be treated with full
confidentiality, and will be used only in aggregate and only for
research purposes.  You will be asked for your permission to link your
ID number with your responses at the end of the survey.  If you
provide permission to make that link, your ID number will be included
with your responses in the file provided by UCLA.  Your ID number will
only be seen in the Institutional Research and Educational Assessment
Office at Smith, and your individual identifiable responses will never
be shared outside this office.  By providing the link to your ID
number, however, you allow us to do more sophisticated analysis of
student responses, by linking responses across years.  These data
never become a part of your student record, however, and your answers
will never be associated with your name.

Please respond to the survey at your earliest convenience.  It will be
available for about the next three weeks.  However, by that time you
will be on campus!  So please take the current opportunity to provide
your responses to this important survey.  Reminder e-mails will be
sent periodically if you have not yet completed it.
