In order for Smith's results to be included in national norms, we must have at least 85% of new students responding to the survey. All of your responses are secure, will be treated with full confidentiality, and will be used only in aggregate and only for research purposes. You will be asked for your permission to link your ID number with your responses at the end of the survey. If you provide permission to make that link, your ID number will be included with your responses in the file provided by UCLA. Your ID number will only be seen in the Institutional Research and Educational Assessment Office at Smith, and your individual identifiable responses will never be shared outside this office. By providing the link to your ID number, however, you allow us to do more sophisticated analysis of student responses, by linking responses across years. These data never become a part of your student record, however, and your answers will never be associated with your name.
