Dear Jagruti,

Within the past week, you should have received a letter from 
President Brodhead inviting you to participate in The Alumni Survey. 
The information below explains how to do this. Completing the survey 
will take only about 25 minutes, and will be of invaluable help in 
enabling us to assess and improve our educational program and 
services for students.

Your participation in this survey is voluntary.  With your 
permission, Duke may match, for its research, your responses to your 
test score information in institutional databases to help analyze 
major and career choice findings, or to other survey responses, such 
as the Freshman and Senior Surveys, to track change over time. This 
information will increase the usefulness of the survey. In all cases, 
the information collected will be kept strictly confidential and will 
not be printed or published in any form that would identify you.  If 
you have questions about the study that were not addressed in Dr. 
Brodhead's letter, please contact me either by email 
(david.jamieson.drake@duke.edu) or by phone at 919-684-0540. If you 
have any questions about your rights as a research participant, 
please contact the Chair of the Duke University Human Subjects 
Committee at 919-684-3030.
