Dear Jag Patel:

Recently, you were asked to complete the Haverford Enrolled Student
Survey. If you have since done so, please disregard this reminder and
accept our sincere thanks. Otherwise, please take approximately 20
minutes to complete and submit the survey according to the
instructions, and encourage your friends to do the same.

There will be several prizes awarded randomly to survey respondents:
an IPod Touch, plus ten $20 awards. The Massachusetts Institute of
Technology (MIT) is administering the survey, and MIT will provide us
with the names of the winners, as Haverford will have no access to
your individual student responses.

This is a unique link assigned to you.

Your voluntary submission of this survey will be considered your
consent to participate, and all responses will remain
confidential. The reporting of survey results will be done exclusively
in aggregate, and no individuals will be identified. Should you decide
not to respond to a question or the entire survey, your decision will
not affect your relationship with Haverford.
