Dear Jag Patel:
 
This will be the last opportunity to provide Haverford with your
opinions and thoughts about your undergraduate experience. If you love
Haverford, let us know or, if you are not satisfied with your
undergraduate experience, we certainly need to know that as well. Your
opinions will be used to improve the undergraduate experience for
current and future Haverfordians.
 
There will be several prizes awarded randomly to survey respondents:
an IPod Touch, plus ten $20 awards. The Massachusetts Institute of
Technology (MIT) is administering the survey, and MIT will provide us
with the names of the winners, as Haverford will have no access to
your individual student responses.
 

This is a unique link assigned to you.
 
Your voluntary submission of this survey will be considered your
consent to participate, and all responses will remain
confidential. The reporting of survey results will be done exclusively
in aggregate, and no individuals will be identified. Should you decide
not to respond to a question or the entire survey, your decision will
not affect your relationship with Haverford.
