There are a number of optional items and opportunities for customization in the 2011 Enrolled Student Survey, which has been designed to have a small core. Please fill out the form below to tell us which items you wish to add, keep, or remove. In each case, the form is pre-set to the "default" survey; most of these are set to "remove." Slots for local items in batteries were added mainly to support local needs to track specific items--please do not feel obligated to use these slots. Please be careful when adding your own text. We will review your submission for obvious errors and overall appropriateness, but you will be responsible for the wording. Links to mockup pages will open in a new window.
