Subject: Enrolled Student Survey Invitation Date: Wed, 2 Mar 2011 12:28:46 -0500 From: The Enrolled Student Survey To: Jagruti S Patel Dear Jag, This year, Smith College is conducting the Enrolled Student Survey. We are conducting this survey to learn about student life at Smith. This is part of a national research project, and we will be comparing Smith to other similar schools. The on-line Enrolled Student Survey consists of nine linked sections. It is important that you complete all nine sections. The entire survey should take approximately 20-25 minutes to complete. To access the survey, please go to the following personal survey URL: [] The survey will be available on-line for only a few weeks. As an incentive for completing the entire survey and completing it early, there will be three drawings of three prizes of $25 in Campus Cash. The prize drawings will begin next week; those who complete their survey early will have more chances to win. We cannot emphasize enough the importance of your contribution to an accurate picture of Smith student experiences and concerns. I want to assure you that your responses will be kept strictly confidential. Your personalized URL will enable us to avoid sending you reminders once you have completed the questionnaire. It will also allow us to link your responses to other information in our files in order to increase the usefulness of the survey. Your name will not be associated with these data, and no individually-identifiable information will ever be reported. I hope I can count on your participation in this important project. The survey is, of course, entirely voluntary. If you decline to participate in the survey, or choose to skip any item, your relationship with Smith will not be affected. There are no physical risks to participating in this survey, but if any item causes you distress or discomfort, please skip that item. If you have any questions about the project, please contact Cate Rowen, Director of Institutional Research & Educational Assessment, at ext. 3021 or survey2@smith.edu. If you have any other concerns about your rights as a research participant that have not been answered by the investigators, you may contact Phil Peake, Co-chair of the Smith College Institutional Review Board at (413) 585-3914. If you have any problems or concerns that occur as a result of your participation, you can report them to the Phil Peake at the number above. Alternatively, concerns can be reported by completing a Participant Complaint Form, which can found on the IRB website at www.smith.edu/irb/compliance.htm. Thank you for your valuable help. Sincerely, Maureen A. Mahoney Dean of the College Smith College
