Subject: Senior Survey Invitation Date: Mon, 06 Apr 2009 12:18:34 -0400 From: Margaret Zelljadt To: Jag Patel Dear Jag, Completing the Senior Survey is an important part of your preparation for graduation. Although you may have already received other surveys this spring, the Senior Survey launches today and I'm asking that you please take the time to complete it. This survey is your opportunity to reflect on your entire Smith experience, voice your opinions, and assist in planning for Smith's future. The online survey consists of several linked sections. The first sections ask you to evaluate your undergraduate education and experiences. Your responses to these questions will be held in the strictest confidence and used only to construct a comprehensive profile of the senior class as a whole. The last section asks for biographical information such as your nickname and preferred class year. The survey will clearly indicate that the information you supply in this final section will go into your alumna record at Smith. The entire survey should take approximately 20 minutes to complete. To access the survey, click on the following link: The survey will be available online until noon on Friday, May 8th. When you submit your Senior Survey, completed or not, you will be sent three complimentary commencement luncheon tickets. Those who do not complete the survey will need to pick up their tickets at dining services. Additionally, as an incentive for completing the entire survey and completing it early, there will be three weekly drawings of three prizes of $25 in Campus Cash. The prize drawings will begin next week; those who complete their survey early will have more chances to win. If you have any questions or encounter any problems, please contact the Office of Institutional Research at survey1@email.smith.edu or extension 3023. Your contribution is crucial for an accurate picture of undergraduate education at Smith. I want to assure you that your responses to the main section of the survey will be kept strictly confidential. Your personalized URL will enable us to avoid sending you reminders once you have completed the questionnaire. It will also allow us to link your responses to other information in our files in order to increase the usefulness of the survey. Your name will not be associated with these data, and no individually-identifiable information will ever be reported, except for information you provide in the section that explicitly states that the data will be stored in your alumnae record. I hope I can count on your participation in this important project. The survey is, of course, entirely voluntary. If you decline to participate in the survey, or choose to skip any item, your relationship with Smith will not be affected. There are no physical risks to participating in this survey, but if any item causes your distress or discomfort, please skip that item. If you have any questions about the project, please contact Kathleen Foley, Assistant Director of Institutional Research & Educational Assessment, at 413-585-3023 or survey1@email.smith.edu. You may also contact Professor Phillip K. Peake, chair of Smith's Institutional Review Board, at 413-585-3914 or ppeake@email.smith.edu. Thank you for your help during this very busy time of the year. Sincerely, Margaret Zelljadt Dean of the Senior Class Smith College This is Smith College official correspondence.
