Filling out the Student Personnel Action form

Here is how Team Athena student employees should fill out the Student Personnel Action Form.

Give the completed form to Jane Trull in E40.

All new watchmakers need to fill this out whether paid, volunteer, credit, or
thesis.

0. Paid students must have an I-9 form on file with MIT. If you don't (or you
don't know) contact the Student Employment office room 5-119, x3-4973.

1. Paid students check "Activation", other students write "volunteer", "credit",
or "thesis" above the boxes.

4. fill in your start date

5. fill in your last name, then your first

6. fill in your ID number

7. fill in your expected year of undergraduate graduation

8&9. fill in citizenship info

10&11. fill in home and school address

12. enter "Student Developer"

13. enter the period (summer, fall, or spring) and year, e.g. summer96, or
spring97.

14. Enter anticipated hours per week, e.g. 35-40, 7-10, or whatever.

15. leave hourly rate blank

16. for job description you can enter "Assist in extending and maintaining the
Athena computing environment."

17. for employer put "MIT Information Systems"

18. for employer's phone number put 253-0119.

19. for employer's address put E40-342G

20-25 leave all these blank

under "Comments".
- put your email address
- your term phone number
- any other phone numbers (e.g. summer, other job, etc.)

Sign the form.

26-28 leave these blank.
