TECHNOLOGY DISCLOSURE FORM
Electronic Version of the Form
This version of the Technology Disclosure Form is a protected Word document.
It allows a user to complete the form using Microsoft Word (minimum version
Microsoft Office 2000). Download the form from the website, save it to your
computer and then complete the form. You can enter data on each of the fields
available for editing. A black box appears or a checkbox appears highlighted
when you are on a field that accepts data. To fill in a checkbox, strike the
“x” key on your keyboard. To uncheck a checkbox, strike the “x”
key on your keyboard. The checkboxes act as toggle switches.
You will note at the bottom of the form that the Inventor’s Signature
line does not allow you to enter data. All MIT inventors must sign this form.
Please print out the completed form and secure the necessary signatures. Forward
the printed form to our office to complete the submission process. Electronic
submissions of this form will not be accepted by our office.
