"X AND Y" & (X) and Y ALSO

1. X and Y
Nouns in Japanese can be combined by using X to Y.
For instance, if you want to say: "Coffee and tea, please."

Koohii-to ocha, kudasai!
OR more POLITELY
Koohii-to ocha, onegaishimasu!
Let's Practice!
Suppose that you're at a restaurant and are about to order the following. Use Polite Order Form (because you are at a restaurant!)

2. (...X) and Y ALSO
When you want to add Y to X, which you have already mentioned in a given discourse, you attach /mo/ right after Y.
For instance, suppose that you already ordered Sushi. But you decided to take Sake, in addition to Sushi.
Then, you say: "....Osake-mo onegaishimasu!"

Let's Practice!
How would you say the following?
