Go here to fill in your entry directory questionnaire. If you are behind a firewall you will not be able to use this program, instead, tell us and we will email you a template which you can fill out and forward back to us.

How To Access Restricted links

To Access any of the restricted links off this page, you need to run the EntryServer. To run the server, login to athena and type:

athena% add e-entry
athena% EntryServer

The server will start and display a URL that might look like:

http://m38-370-17.mit.edu:41357

Point your browser to this URL (make sure the numbers after the hostname match). Now you will have access to all the links. Note: the server must keep running for you to have access to the restricted stuff - the moment the server is killed, access stops.

How to Administrate

Go to help to learn how to use the admin program. This allows you to change the list of people who are allowed to fill in the questionnaire, change the list of questions on the questionnaire, make a web version of the entry directory, and view/print the final product.
