Emergency Contact

Identify the emergency contact person for the employee. Other required fields are the relationship and at least one telephone number where the emergency contact can be reached.

1. Type in the person's first and last name.

2. Enter their relationship to the employee (spouse, brother, etc.).

3. Enter the home, work, and mobile telephone number of the contact. You can enter all three, but at least one is required.

Optional fields for the emergency contact person:

4. Enter the work or home e-mail address.

5. Select the country where he/she lives.

6. Enter the home street address in Address Line 1. Use Address Line 2 for additional address info such as P.O. Box number.

7. Enter the city.  If the country is not US or Canada, also enter the region (prefecture, territory, etc.).

8. Select the state (US) or province (Canada).

9. Enter the zip or postal code.

10. Click Save to save what you have entered, or click Return to cancel your changes and return to the “Search Results Grid” page.

11. After saving, click Return to go back to the “Search Results Grid” page.
