Each week in 6.1800 you'll have a paper to read. With the exception of this first week, you should read the paper before the first lecture of that week, unless otherwise stated (we will post the papers in advance on this very website). It is okay if you don't understand everything in the paper during your first read! It is normal to skim through some of the more confusing parts and come back to them later.

For this week, read We Did Nothing Wrong: Why Software Quality Matters by Deborah Gage and John McCormick. This paper details a number of software systems that failed in spectacular ways, with most of the focus on a radiation machine made by Multidata called the Cobalt-60.

As you read, think about the following:

  • How did the Cobalt-60 fail? Why did it fail? Who was impacted by its failures?
  • What fault(s) contributed to the failure of the Multidata machine?
  • Are there quality-control procedures that might have made a difference?
