In order to determine which forms you need to complete, please answer all the questions.

Student Background Information:
Are you under the age of 18?
Are you a Transfer Student?
What is your Student Type?
When did you initially enroll at MIT?

MIT Varsity Sports Participation Information:
Have you previously participated in a varsity sport at MIT?
Do you plan to join Crew/Rowing? If Yes, Select your Gender
Do you plan to join a varsity sport OTHER than Crew/Rowing? For a listing, click HERE

Determination of International Status:
Are you a citizen of a country OTHER than the United States?
Did you ever reside in an unincorporated or commonwealth territory of the United States?
Did you ever participate in foreign athletics activities or were associated as an athlete with a foreign athletics organization?

Please be sure printer paper tray is full. All forms must be printed, filled out, appropriately signed and postmarked by August 1, 2012.
