In late August and early September of 2007, the "baseline" survey was undertaken in the 500 evaluation villages. In each village, four separate surveys were administered: (1) a Male Head-of-Household Questionnaire (MHH-Q), administered to ten randomly selected male heads-of-household; (2) a Male Focus Group Questionnaire of Village Leaders (MFG-Q) administered to between six and nine village elders or leaders in each village; (3) a Female Focus Group Questionnaire (FFG-Q) administered to between six and nine women in each village; and (4) a Female Individual Questionnaire (FI-Q) to be administered individually to the women that participated in FFG-Q. To provide guidance to the enumerators, three sets of enumerator manuals (for the MHH-Q, MFG-Q, and for the female questionnaires) were written. The guidelines explained the meaning of each question and provide guidance to enumerators in the event that respondents provided ambiguous responses.
