Format for local questions
Word / text file with up to 25 optional closed-ended questions with up to 6 responses, and 5 open-ended questions at no additional charge. The MIT Web Survey Service can design alternative formats for additional charges, negotiated separately.

Format for sample file
One parent per row with the following columns:
SCHOOLID FOR PARENT, EMAIL, FIRSTNAME, LASTNAME, STUDENT ID, OTHER2, OTHER3
The two other fields are optional, but allow schools to include a few more demographic variables if desired.

Notes:
mockup = mockup for review by school
approved = mockup approved by school contact
coded = on live server
live = launched to students
closed / data = survey closed, data downloaded
